About the Opportunity

A healthcare company in New York City is currently seeking an experienced, licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) to join their growing staff as their new Advanced Practice Clinician. In this role, the Advanced Practice Clinician (NP / PA) will be responsible for working with a panel of members to improve their health holistically through longitudinal primary care, care management, and care coordination. Job Description

The Advanced Practice Clinician (NP / PA) will:

  • Perform episodic urgent medical/behavioral health visits and/or telephone calls for members on the panel to ensure that timely and appropriate medical care in order to avoid emergency department visit or hospitalization
  • Perform post-discharge visits for the panel within 48 hours of discharge from either an acute care facility or skilled nursing facility to decrease risk of readmission
  • Perform detailed medication reconciliation and assure that appropriate long term services are in place
  • Monitors and addresses clinical quality gaps in care
  • Attend external meetings  activities and maintain professional credentialing and CME standards
  • Assist with other duties, as needed

Required Skills

  • 2+ years of experience in a Community Health, Home Care, and/or Primary Care role/setting
  • Bachelor's Degree in a related field
  • NYS Nurse Practitioner (NP) and/or Physician Assistant (PA) license
  • AANC or AANP certifications
  • BLS certification
  • NYS Controlled Substance and DEA Controlled Substances registrations
  • Experience providing Clinical Services to individuals with Co-occurring Chronic Medical and Behavioral Health conditions
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Desired Skills

  • Bilingual
  • Experience as a Geriatrician or Behavioral Health Specialist in a Low-Income community or in a Community Health setting
  • Experience working collaboratively with an Interdisciplinary Care team and specifically working alongside Community Health Workers or Care Coordination staff
